The Health Promotion Program:
- The health problem: Identify what health issues need to be addressed. It could be heart disease, diabetes, obesity or other health issues.
- Evaluate the community. Conduct a community survey to determine the resources and needs of the community to address any health concerns. The community’s demographic and socioeconomic data will be analyzed.
- Create goals and objectives: Based upon the community assessment, create specific goals and measurable objectives for your health promotion plan.
- Create strategies and activities. Develop strategies to reach the objectives and goals of your health promotion plan. You could develop educational materials or organize health fairs.
- Execute the plan. This involves executing activities and strategies that were developed in step 4. It may include working with stakeholders and community partners.
- Evaluation of the plan: Measure progress toward the goals and objectives and analyze community feedback to determine if adjustments are needed.

- Construct the session: Use a range of teaching methods such as group discussions and lectures to conduct the education session.
- Assess the session. Collect feedback from target audiences and evaluate how effective the educational session was. This will allow you to measure the progress towards your goals.
