Stetler Model, a well-respected evidence-based practice model, includes five stages. These are preparation, validation and implementation. Evaluation is then followed by dissemination. The following are methods to track the execution of the Stetler Model-based patient-centered plan in an interdisciplinary meeting:

1. The preparation stage: This stage is where the inter-disciplinary team first identifies and then develops a solution using the evidence available. This stage allows the team to create a monitoring plan which will be executed during the implementation phase. This should contain:

• To evaluate implementation, identify the key indicators to be tracked
• How to determine the frequency of monitoring, and data collection methods
• Responsibilities for reporting and monitoring data to the group
• Develop a communication strategy to communicate progress and resolve any issues during monitoring

2. The validation stage: Before implementation, an inter-disciplinary team assesses whether the proposed solution is feasible and effective. You can validate the monitoring plan by:

• To identify potential problems or difficulties, conduct a pilot testing of the monitoring program
• Collecting team feedback regarding feasibility and utility of the monitoring program
• Ensure that the monitoring plan is in line with the goals of the patient-centered care plan

3. Implementation phase: This is where the inter-disciplinary team actually puts into action the solution. This is how the monitoring plan could be put into practice:

• Monitoring plan: Regular data collection based upon the monitoring plan
• Assure that the team follows the monitoring plan
• Resolving issues during monitoring
• Based on data from the monitoring, updating the patient-centered care plan

4. Evaluation Stage: The evaluation stage is where the multidisciplinary team reviews the data collected throughout the monitoring process in order to evaluate the effectiveness of the solution. You can evaluate the monitoring plan by:

• Analysis of the collected data to detect trends and patterns
• Compare the results to the baseline data from the preparation phase to see if the solution is achieving the expected effect.
• Collect feedback from the team about the effectiveness and usefulness of the monitoring plan

5. Dissemination Stage: The interdisciplinary team discusses the monitoring results with healthcare providers and stakeholders during the dissemination stage. This is how the monitoring plan may be distributed:

• The creation of a summary report that summarizes all the observations from the monitoring process
• Share the report with healthcare providers and other stakeholders in order to share best practices and improve patient outcomes
• To ensure it’s evidence-based, and efficient, incorporate the results into the patient-centered care plan.

The Stetler Model is a system that monitors the implementation of evidence-based, patient-centered care plans. By following the steps outlined in the model, interdisciplinary teams can ensure that the patient-centered plan of care is effective and aligned with the patient’s goals and objectives.
