This requires engaging with team members on all levels—including nurses, patient care assistants, social workers, unit leaders, etc.—to brainstorm ideas that can reduce avoidable ED visits without sacrificing quality of care. By encouraging ownership over the problem solving process and gathering suggestions from frontline staff who are most likely experience with patients’ day-to-day needs at first hand – new interventions & protocols can be developed & designed specifically for nursing home setting targeting reduction within short term . Open communication channels between departments must be maintained so that any changes proposed are clearly communicated before they can be implemented.  .
This could be done well and help to break down silos across the entire system. It also helps build proactive action plans that focus on both intervening early, when necessary, while avoiding congested emergency rooms.
