Progress Note: This was the fourth therapy session for client families. Cognitive-behavioral Therapy was used as the treatment method. It appears to have been effective in helping families understand their problems and resolve them. Although progress has been made towards the mutually agreed-upon client goals as set out in the treatment plan there were some adjustments made because of insufficient progress in particular areas. Clinical impressions show that the client family is making strides towards understanding each other’s perspectives and starting to empathize with one another’s views on various topics discussed during sessions. The following psychosocial changes or information are relevant to the client family: a move to an apartment/house, a change in job, and additional stressors regarding schooling during times of pandemic. These factors can impact motivation levels. Safety issues have not been reported. All members of the family have been consistent with their treatment since the first sessions began. No clinical emergency or action has been taken thus far. Consultation with a physician was required prior to beginning sessions. It is recommended that clients transition into couple counseling after completion of individual therapy. If insurance coverage is canceled, consent was granted. Patient agreed to termination planning. No signs evidence abuse reported . This record shows that the therapist was able to exercise sound clinical judgement during all discussions.
