For an initial interview with a client, I use the following parts: Introduction phase; Presenting problem/Reason to Refer; History of present Illness; Mental State Exam (MSE); Closing Phase.
The introduction phase: At this stage it’s important to present oneself politely and clarify their role in caring for others. Also, explain what the assessment is for and how it will be completed. In preparation for this discuss the patient’s expectations around privacy and confidentiality in addition to building rapport with him or her.
Referral – Presenting problem/ reason for referral: Provides essential information such as the reasons why you have been referred, any known problems or any other symptoms that may be contributing. Additionally, any supporting medical conditions should be discussed during the part evaluation. Before moving on to the next stage, it is crucial that all processes are clearly explained. This will ensure they receive the best quality of care.
Histories of present illness: Provides additional information about current complaints by reviewing dates, duration and frequency. This allows for a better understanding of the triggers that are being experienced. Reported symptoms can either be caused by mental or physical illness. These questions should be answered honestly so that one can determine the underlying cause of illness and create a treatment plan to manage it.
The Mental Status Exam (MSE). This allows for further evaluation to determine the severity of individual symptoms such as cognitive accuracy, attention and concentration. Other medical conditions can also be detected differently depending on who is looking.
